You're a customer service agent and somebody called the number of your client which is supposed to be confidential information. What will you do?
If that would happen, that's a real serious problem. I would ask someone who called my client to apologize and ask how did the person knew that confidential information.

Your boss is very busy and you didn't understand his instructions clearly. How will you clarify it to him?
I really don't like this kind of situation. But if I didn't understand clearly, I cannot do anything in progress. So I am going to make a direct call and ask my boss again what was the exact instruction.
